The RN may check in periodically, answering medical-related questions within scope and evaluating whether additional supports might be helpful.
Alongside the nurse, each family is assigned a care coordinator who becomes their primary point of contact. Coordinators assist with scheduling, paperwork, timesheets, communication and problem-solving. They connect families with resources and help navigate available care options, keeping the process manageable and stress-free.
The coordinator and RN work together to update care plans over time, adjusting for new needs or preferences. They support family caregivers by offering guidance, answering questions and ensuring the individual receiving care remains at the center of every decision. This ongoing collaboration ensures a smooth, well-managed experience that blends professional oversight with family-directed support.

Starting CDS with HomeTeam is simple. First, individuals must meet eligibility requirements, including being on Missouri Medicaid and requiring assistance with activities of daily living.
